Travel Permit Request (OMHA)
All teams (OMHA) are required to have a valid travel permit in order to attend any tournament or play an exhibition game. Please complete this form once your tournament entry has been approved or your exhibition game has been officially scheduled. The OMHA Rep will submit the request for approval. Once approved, a copy will be sent to the Team Contact listed. All questions or urgent requests can be directed to the OMHA Rep by email at [email protected].
  • I understand I must submit a travel permit request at least 7 days prior to the tournament or exhibition game.
  • I understand travel permits for tournaments or exhibition games are not granted until OMHA paperwork has been received.
